Job summary

We are seeking a dedicated professional to join our Community Specialist Palliative Care Nursing Team. This is an exciting opportunity to provide clinical leadership as part of a supportive, highly skilled service that provides compassionate, person-centred care to palliative patients and their families at a crucial time in their lives.

As a valued member of the team, you'll play a vital role in delivering specialist palliative care within the community, providing clinical leadership to less experienced team members and workingcollaboratively with colleagues across health and social care. You'll bring not only your clinical expertise and leadership but also warmth, empathy, and a commitment to making every contact count.

Main duties of the job

Community specialist palliative care nurses work closely with the multidisciplinary team and with other specialist palliative care providers.

The post holder will manage a geographically defined caseload alongside a caseload of very complex patients to provide guidance and leadership to less experienced team members and will ensure that all patients referred with complex palliative needs receive high quality symptom assessment, psychological support and specialist advice to meet their individual care needs.

The post holder will use recognised referral guidelines to undertake agreed levels of intervention.

The post holder will demonstrate high levels of clinical decision-making with excellent communication and team working skillsand will monitor and improve standards of care through supervision of practice, clinical audit, research and by teaching and providing support to professional colleagues.

The post holder will participate in a rota providing a seven-day service.
