Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity for an experienced Nurse or Paramedic to join the Reading Urgent Community Response and Virtual Ward Team.

You will provide advanced clinical assessment and patient-focused care to people in their own homes, including care homes, who require an urgent response to help prevent unnecessary hospital admission.

Working as part of a multidisciplinary team, you will provide clinical leadership, line management, supervision and training to junior staff, including nurses, therapists and support workers.

We are looking for a confident, compassionate and organised clinician who works well with colleagues across disciplines, maintains high professional standards and supports effective communication across the service.

This is a part-time role of 25.5 hours per week, covering a 7-day rota between 8am and 8pm, including weekends and bank holidays.

The role is based at Bath Road, Reading, with free staff parking available on site.

Main duties of the job

  • Practise at an advanced level as an autonomous practitioner within the multidisciplinary team, supporting admission to and discharge from the Urgent Community Response Service in collaboration with the Community Geriatrician and GP.
  • Undertake advanced clinical assessment, diagnosis, treatment and management of patients with complex needs, using local and national guidance.
  • Communicate professionally with colleagues, external organisations, patients and the public, ensuring confidential information is managed appropriately.
  • Provide leadership and line management to Band 6 nurses and Band 4 Support workers.
  • Maintain accurate, contemporaneous documentation and care plans in accordance with NMC, information governance and local and national guidance.
  • Work collaboratively with colleagues to support service development, care delivery and policy implementation.
  • Work flexibly in response to service need across multiple sites.

Job description

The 'must haves for this role:

  • Registered General Nurse or Paramedic.
  • Advanced history-taking and physical assessment qualification or working towards this.
  • Non-medical prescriber qualification, working towards this or willing to undertake within 12 months of appointment.
  • Able to work autonomously and under pressure, with excellent organisational, communication and time management skills.
