Community Ageing Well Service (CAWS)

We are looking for an enthusiastic and motivated Specialist Occupational Therapist to join our innovative Community Ageing Well Service (CAWS) on a fixed-term maternity cover basis.

Working as part of a multidisciplinary team, you will play a key role in supporting older people with frailty, complex health needs, and long-term conditions to remain independent in their own homes. You will have the opportunity to provide expert occupational therapy assessment and intervention, helping individuals maximise their function, confidence, and quality of life.

About the Service

The Community Ageing Well Service is a multidisciplinary service dedicated to supporting older adults through proactive, preventative, and holistic care. We work closely with GPs, Community Rehabilitation Services, Social Care, Care Homes, Voluntary Sector organisations, and Integrated Neighbourhood Teams to deliver coordinated person-centred care.

Our focus is on:

  • Promoting independence and wellbeing
  • Preventing avoidable hospital admissions
  • Reducing falls and functional decline
  • Enabling people to remain safely in their own homes

Main duties of the job

As a Specialist Occupational Therapist, you will:

  • Undertake comprehensive assessments of patients with complex physical, cognitive, and functional needs.
  • Develop and implement personalised treatment and rehabilitation plans.
  • Assess for equipment, adaptations, and moving and handling requirements.
  • Support patients to achieve their goals and maximise independence.
  • Work collaboratively within a multidisciplinary team, contributing to holistic care planning.
  • Provide specialist advice and support to patients, carers, and healthcare professionals.
  • Participate in falls prevention, frailty management, and admission avoidance initiatives.
  • Supervise and support therapy assistants and junior staff where appropriate.
  • Contribute to service development, quality improvement, and clinical governance activities.
