Job summary

Join CLCH Core Community Therapy in Hertfordshire as an Occupational Therapist and develop your clinical, rehabilitation and management skills while making a meaningful difference to people in their own homes.

You will work with a varied caseload, supporting people with prevention of admission, supported discharge and community rehabilitation. This includes people living with frailty, neurological conditions, falls and balance difficulties, orthopaedic conditions, and those receiving palliative or end-of-life care.

You will help people maximise their independence, participate in everyday activities and improve their quality of life at home. You will assess and provide occupational therapy interventions, including provision of equipment, and work in partnership with people, families, carers and colleagues across health and social care.

This established, supportive team offers an excellent opportunity for an Occupational Therapist from acute, bed-based rehabilitation or social care to develop community experience on a part time basis.

A full valid driving licence and regular access to a car for work are essential, unless you are unable to drive because of a disability as defined by the Equality Act 2010.

Main duties of the job

As an Occupational Therapist, you will:

  • Assess, plan and deliver occupational therapy interventions in people's homes, helping them optimise functional ability and independence.
  • Screen and triage new referrals to the Therapy Service and contribute to safe, effective prioritisation of care.
  • Provide equipment and practical recommendations that support people to remain safe and independent at home.
  • Work autonomously while collaborating closely with Occupational Therapists, therapy assistants, physiotherapists and the wider multidisciplinary team.
  • Supervise Band 3 and Band 4 therapy staff, supporting their development and effective practice.
  • Work with community nursing, complex care and frailty services, GPs, palliative care specialists, hospices, social care and voluntary and community organisations to provide coordinated care.
  • Travel across the locality to visit people in their homes and, where required, transport equipment from satellite stores.
