Job summary

Help people return home safely, confidently and with the right support in place. As a Safe Discharge Therapist, you will play a vital role in promoting independence, reducing risk and supporting adults to achieve the best possible outcomes after a hospital stay.

This role includes an expectation to work from our Time Square office and hospitals, three to four days per week, alongside flexible working arrangements.

You will work within the Hospital Discharge team while maintaining a broad remit across Adult Social Care, including close working relationships with Community Mental Health teams, Learning Disability teams, Early intervention and Prevention services and Adult Community teams.

We know there can be pressures working in Adult Social Care, so we make sure our staff are well supported, receiving regular supervision and access to training. We have a strong focus on supporting our staff to learn, grow and develop to achieve their potential.

Main duties of the job

About youWe are looking for a qualified Occupational Therapist or Chartered Physiotherapist with current HCPC registration who brings a person-centred, strengths-based and outcome-focused approach to practice. You will be flexible, proactive and open to learning, with the confidence to make sound decisions while managing complexity and risk. Above all, you will be committed to promoting independence, dignity and wellbeing, supporting people to achieve safe and positive outcomes following hospital discharge.

Key responsibilities

  • Undertake holistic functional assessments after restarts of care packages, including return to existing packages at home or re-entry into care home placements after discharge from hospital.
  • Complete detailed moving and handling assessments, ensuring risks are identified and mitigated to prevent any readmissions.
  • Assess activities of daily living (ADLs) and functional abilities to determine safe levels of independence and support required.
  • Support and facilitate Home First approaches, prioritising return to home wherever safe and appropriate.
  • Identify when a temporary increase in care, reablement or rehabilitation provision is required to support recovery.
