Main duties of the job
The post holder will provide high-quality, patient-centred clinical care with a particular focus on home visiting, frailty, admission avoidance and supporting patients with complex or uncertain presentations. Working autonomously and as part of a multidisciplinary team, the Salaried GP will assess, diagnose, manage and review patients in the community, making safe clinical decisions and escalating care appropriately where needed.
The role will also involve providing clinical support and guidance to colleagues within the PCN team, including advanced practitioners, paramedics and other healthcare professionals. The successful candidate will contribute to multidisciplinary working, clinical governance, service development and quality improvement, helping to strengthen community-based care and ensure patients receive the right care in the right place at the right time.

KEY DUTIES TASKS AND RESPONSIBILITIES
Service Development, Delivery and Ways of Working
Work with the PCN Clinical Directors, PCN Manager, Team Up Operational Lead and other senior clinical leads within the PCN to develop and implement new care pathways for the service areas of acute home visiting, urgent community response, enhanced health in care homes, anticipatory care and other PCN led clinical services that supports the multi-agency and multi skilled team approach.
Developing clinical leadership is an important part of our team culture. The successful applicant will have opportunities to build confidence in leadership through multidisciplinary working, supporting colleagues, contributing to governance and quality improvement, and becoming involved in service development.
As part of the role, you will also provide clinical support to the PCN Team working in Ragsdale House. The team is made up of ANPs, Paramedics, Social Prescribers, FCPs, Mental Health Practioners and Health Care Support Workers.
Support the teams in identifying and responding to safety and quality concerns, ensuring these are escalated as appropriate.
Providing medical assessment and management where needed on behalf of, or in conjunction with, team members.
Work with the Team Up operational lead and Home Visiting Service clinical lead(s) to develop and implement (with appropriate clinical protocol) the senior clinical triage function for the Home Visiting element of the service to determine the urgency and type of response according to clinical need.
