Job summary

Wellspring Surgery is recruiting a part-time (15-20 hours) Clinical Pharmacist who has a keen interest in providing long term conditions management with our patients. You will be able to practice independently and be a valuable part of the team. Applications are welcome from Clinical Pharmacists both with and without the Independent Prescribing qualification.

The purpose of the role is to support the general practice workforce and improve patient care. We are keen that this role develops in line with the national Clinical Pharmacist programme. Ideally the postholder will have completed the CPPE pathway, but the surgery will support the right candidate to achieve this: Primary care pharmacy education pathway : CPPE.

Main duties of the job

Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas

Supporting patients with chronic disease management

Undertaking clinical medication reviews

Managing prescription processes and medication queries

Providing clinical leadership on medicines optimisation and quality improvement, including QOF

Development support and mentorship will be provided on a practice level and across the PCN

You will be working alongside other Pharmacists and a Pharmacy Technician

Liaises with ICB colleagues including ICB pharmacists on prescribing related matters to ensure consistency of patient care and benefit

Liaises with ICB pharmacists and Heads of Medicines Management - Optimisation to benefit from peer support

Leadership On person centred medicines optimisation

Supporting further integration of general practice with the wider healthcare teams

Liaises with other GP practices and staff as needed for the collective benefit of patients

Demonstrates understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace

Demonstrates understanding of, and contributes to, the workplace vision.

Demonstrates ability to improve quality within limitations of service

Reviews last years progress and develops clear plans to achieve results within

priorities set by others

Demonstrates ability to motivate self to achieve goals

Management Demonstrates understanding of the implications of national priorities for the team and service

Wellspring Surgery comprises 3 GP Partners, 9 Salaried GPs, Clinical Pharmacists, Advanced Practitioner, Nurse team,First Contact Physio, Mental Health Practitioner, Podiatrist and a friendly administration team. We always aim to provide a positive experience for our patients.
