Main duties of the job

  • To work within the scope of the New Model of Care and undertake Early Days In Custody (EDic) assessments. To ensure all new arrivals to receive a brief assessment of individual presentation and need, referring patients on to other services (GP, SMS, chaplaincy, psychology etc) and book further assessment if indicated.
  • To participant in Care and Separation (Segregation) Unit (CSRU) rounds with a GP and primary care nurse.

To provide professional leadership and to facilitate a culture which engenders highstandards and reflective learning

To lead and manage a team of allocated staff and respond to emergency on site: code blue or code red

Undertake line management responsibility for a team of staff ensuring appropriate deployment and supervision of staff

To provide skilled general care and advice to patients within the prison

To work as part of a multi-disciplinary team assessing the health needs of patients; toprovide and evaluate care given and implement changes as required

To be able to work autonomously, as well as part of the wider team delivering health care to the standard equitable to that found within the wider community

To provide holistic care in a unique environment based on NMOC that

encourages patient engagement to an often socially excluded group

The postholder is expected to deliver trauma-informed, culturally sensitive care.

Job description

  • Assume responsibility for the Paramedic response with and without the direct supervision of

senior healthcare managers, to include the assessment of care needs and the provision

and evaluation of individual patient programmes of care

  • Act as first response to medical emergencies providing first aid / emergency care as

required

  • Establish and manage care packages including assessment, planning, monitoring and

evaluation of care

  • Support Healthcare Care colleagues in the provision of care within the prison to prisoners with long term conditions e.g. asthma, CHD
  • Apply evidenced based knowledge and the New Model of Care in quality care delivery
  • To assess prisoners presenting with minor injuries and illnesses as part of a triage process
  • To effectively and confidently treat prisoners in line with agreed local policies
  • To work autonomously within a nurse lead service, recognising when there is a need to

refer to other members of the team

  • Effectively communicate treatment plans with nurses and other members of the multi

disciplinary team

  • Provide immunisations and health promotion
  • To work with prisoners in order to support concordance with prescribed treatments
  • To assist in the development of staff and to commit to ensuring a high standard of care is

provided safely and within all local policies and guidelines in relation to IPC and health and

safety

  • To ensure all patient care is recorded accurately (on System One) and clinical decisions

are based on best practice and are evidence based (e.g. NICE, NSF)

  • Attend ACCT reviews and other multi-disciplinary meetings as required, including complex case review meeting
  • Screening and assessment of patients on reception including the completion of all relevant

documentation

  • Maintain clinical competency in a range of extended skills as required to meet the needs of

patients on the caseload

  • To act as an advocate to facilitate the views and choices expressed by patients
  • Work with other health care colleagues, voluntary and statutory organisations to ensure that the holistic needs of patients are met wherever possible
  • Administer medication including controlled drugs in accordance with national guidance and

Local Operational Procedures (LOPs) and Standard Operating Procedures (SOPs)

  • To assume responsibility and accountability of a designated unit of the prison in relation to meeting the healthcare needs of patients along with making sure care plans are updated regularly.
  • Working with the Clinical Team Leader to provide an environment that promotes training and development among the workforce.
  • To assess health and social care needs and negotiate care plans with patients and staff teams.
  • To ensure that safeguarding procedures are followed.
  • The post holder where possible will, under the direction of the team leaders and the MDT, ensure that the service is delivered to patients inclusively and effectively.
