We have an exciting opportunity for a Medicines Optimisation Pharmacist to join our Pharmacy team covering the South Yorkshire Region.
This role covers HMP Doncaster, HMP Lindholme, HMP Moorland and HMP Hatfield which are all located in South Yorkshire and you will be required to work across all sites. Hours
37.5 hours per week, Monday to Friday, core business hours.
Salary
This position is full time, youll receive an annual salary of £51,643 to £57,955 per annum - depending on experience.

Main duties of the job
As a Medicines Optimisation Pharmacist with Practice Plus Group you will; Provide expert clinical advice on medicines and poly-pharmacy in line with expected standards, ensuring that safe, modern, medicines management and medicines optimisation service are provided to patients.
Deliver the national company strategy around medicines optimisation and Structured Medication Reviews which will be part of a comprehensive healthcare package based on the prison population needs.
Inspire positive and aspirational leadership to the team of pharmacists and pharmacy technicians and support the development of both to ensure high standards of clinical expertise are available locally at each site.
Promote good practice in all prescribing and optimisation decisions while ensuring patient engagement remains as a focus of the medicines optimisation agenda.
Share information relating to medicines optimisation and good evidence-based practices to encourage high standards and sound clinical decision making processes.

Qualifications & Experience required
- GPhC registered Pharmacist
- Non-Medical Prescriber
- Available to newly qualified pharmacists and beyond
- Management and leadership experience
- Procurement and audit experience
- Vocational Masters Degree in Pharmacy including pre-registration training
- Excellent communication skills
- Experience developing effective working relationships with internal and external stakeholders
- Non-judgemental and compassionate approach
- Good IT skills and IT literacy
- Ability to work within a busy environment
