Job summary

An opportunity has arisen for a experienced Nurse to join our multi disciplinary team to work as part of urgent community response delivering high quality nursing care in patient's homes across West Suffolk.

The role requires the successful candidate to manage a caseload of patients and support the management of patients with multiple and complex needs, using evidence-based practise to assess, plan, implement and evaluate interventionsTo be responsible for the day-to-day co-ordinating, planning and delivery of the nursing servicesTo work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge.Initially the role will focus on urgent community response but will expand into general community nursing.

Main duties of the job

Be able to complete a holistic assessment of any new patient, taking into account their physical, social, psychological needs.Be able to plan, carry out and evaluate treatment interventions for the patients and modify healthcare plans as needed, using persuasive, motivational, negotiating, emphatic and reassurance skills.Be able to encourage patients towards self-care and independence, and provide training, education and advice, using a health coaching approach.Aim to avoid admissions to the acute setting, where possible, and actively support discharges into the community, using problem solving approach. Manage and oversee the day to day triage of referrals to the team.Be responsible for the coordination of the day to day planning and delivery of nursing services, prioritising clinical needs and delegating where needed. Utilise risk management strategies and good communication skills to effectively manage the workload and follow the escalation policy.Be able to reflect on ethical and moral issues and provide guidance to others, using an effective leadership style.Support multi-disciplinary communication and work collaboratively with the team, patients, carers and other health workers and agencies involved in patient care.Promote best clinical practice as per agreed national, professional and local standards and policies

As this role is supporting patients in their homes, you will be required to travel across West Suffolk. As frequent travel is essential for this role, therefore you must hold a full UK current driving licence. You will be asked to show a copy of your original driving licence at interview.
