Job summary

The Patient Safety Incident Response Framework (PSIRF) has now been implemented within Sandwell and West Birmingham Hospitals NHS Trust (SWBH) for two years and our Patient Safety Investigators play a key role in the continued embedding and development of our processes. We are looking to recruit an enthusiastic individual with a passion for improving the safety and experience of Our Patients, People and Population to join our team and help us continue to learn and improve.

As a Patient Safety Investigator within the Corporate Patient Safety Team, you will work closely with the Clinical Divisions to promote a culture of openness and learning from good practice and areas that may need some improvement. You will lead on a range of incident investigations looking at systems and processes using the SEIPS tools and support clinical staff to understand and use a range of learning tools including swarm huddles, After Action Review and Multidisciplinary Team reviews.

Supported by the Quality Improvement team you will be part of a team to recommend and implement improvements across the Trust, working across a wide range of teams. You will also engage with patients and their families to understand their perspective as part of your investigations, ensuring their experience is at the centre of all improvements made. Main duties of the job

You will be working closely with clinical divisions, across a wide range of specialties and clinical grades. You will need to be able to adapt your communication style to engage in discussion with team members, showing empathy and compassion during difficult conversations when things may have gone wrong. You will equally need to demonstrate these qualities when engaging with patients and families who may have been part of an incident.

In addition to excellent verbal communication, you will also need to demonstrate effective written/digital skills to provide appropriate assurance and updates regarding your areas of responsibility on a regular basis.

To be successful in this role, you need to have a natural curiosity and want to delve into systems and processes to understand how things may need to be improved, or how a process may be adapted to suit a different area of practice.

Analysis of data in various formats will be required in order to horizon scan and anticipate any issues before they are encountered, or recognise when an issue may be emerging.

There is also opportunity to create and deliver learning materials in various formats to your local areas, engage in research for patient benefit and support the development of policies.
