Job summary

We are seeking an enthusiastic Clinical Pharmacist who is keen to develop their skills in primary care as part of our pharmacy team. This role offers the opportunity to work closely with Senior pharmacists, GPs and pharmacy technicians while developing expertise in medicines optimisation, long-term condition management and prescribing safety.

Main duties of the job

As a Clinical Pharmacist, you will work with the pharmacy team to support medicines optimisation across the PCN. You will undertake structured medication reviews, support patients with long-term conditions, participate in prescribing safety initiatives and provide expert medicines advice to patients and colleagues. You will work closely with Senior Clinical Pharmacists to develop skills in long-term condition management, quality improvement and clinical leadership. You will also have the support of pharmacy technicians and a dedicated pharmacy administrator. If you have not already completed the CPPE PCEP course, you will be enrolled onto this shortly after joining us.

Key responsibilities include

Undertake medication reviews and structured medication reviews.

Support the management of patients with hypertension, hyperlipidaemia and other long-term conditions.

Support the achievement of QOF and prescribing quality scheme (PQS) targets.

Participate in audits and quality improvement, including patient safety alerts.

Participate in pharmacist-led duty clinics, responding to on-the-day medication queries.

Review high-risk medicines and monitoring requirements

Manage complex medication reconciliation post hospital discharge.

Provide medicines advice to patients and healthcare professionals.

Work closely with senior clinical pharmacists to develop specialist clinical knowledge.
