Job summary

We are currently seeking an experienced Support Worker, at Band 4 to work via a Bank Contract, based in Bishops Stortford for our Community Adult Mental Health Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

The post holder will work as part of a community team which provides mental health services and support the needs of carers requiring support (including assessment of their needs) and act as an expert within geographical area of responsibility.

You will support service users to have their social care needs assessed in the Connected Lives model, access universally available resources and put in place services with direct payments/personal budgets and focus directly on the needs of service user using the Support Time and Recovery Model.

HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

Main duties of the job

To work within the Care Programme Approach Care Management process with a focus on Recovery Principles, supporting the development of personalised care and support plans.

To support community team members in understanding best practice approaches to assessing the needs of and supporting carers.

To work within approaches outlined through the Care Act 2014 and local policy to ensure timely access to support for carers of people with a mental health issue.

To assist the Senior Social Worker to ensure the integrity of the HPFT Carer Pathway within geographical area

To ensure that the wellbeing of carers and service users is maintained at all times through promoting recovery-oriented practice.

Job description

To carry out direct work with carers including:

o Carer assessments and reviews

o Carer contingency plans and wellbeing plans

o Providing information and support to promote and enable carer recovery

o Holding a caseload of carers,

o Supporting carers to develop coping skills and achieve their well-being goals

o Accompanying carers to appointments / meetings and performing a range of practical tasks, relating to their identified needs

o To support carers to understand the recovery goals, care plan and advanced directives and/or statements of the person they care for and to be involved in planning and implementing these

o Support carers to understand their entitlement to a carers assessment and the support that is available to them from HPFT and other services

o To facilitate access to community groups and networks that enable carers to participate in community activities, in order to maximise their opportunities to have a positive identity and socially valued roles
