Job summary

We are currently recruiting for a Community Wound Care Nurse to join our busy yet rewarding team.

The Community Wound Care Nurse will be responsible for leading wound / pressure care management across Bracknell Community Nursing Service. They will be an expert resource for community staff. The post holder will ensure the delivery of evidence-based care in relation to all aspects of Tissue Viability including leg ulcer management. They will improve and maintain standards of patient care through the implementation and continuous evaluation of quality improvement initiatives.

You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community.

You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.

Main duties of the job

  • Assess current treatment practices, maximise cost effectiveness whilst improving outcomes.
  • Provide information and advice to healthcare professionals about tissue viability using enhanced clinical judgement. Act as an information resource on tissue viability issues.
  • To support the provision of education and training relating to tissue viability to all levels of staff and disciplines.
  • Support and contribute to the development of programmes of care to improve clinical outcome in the prevention and treatment of pressure ulcers, leg ulcers and non-healing and complex wounds.
  • To support the effective and efficient management of pressure relieving aids and equipment across the Trust in conjunction with equipment specialist nurse.
  • To support the development and implementation of systems for the audit of pressure ulcers, leg ulcers and complex wounds
  • Your working hours 08:30-16:30

Job description

The must haves for you to be considered for this role:

  • Leg ulcer/wound care qualification.
  • A passion for community nursing, that matches our passion to provide excellent healthcare for our patients within their own home with complex wound.
  • Adult nursing qualification along with NMC registration
  • A full UK Driving Licence and have use of a car for work purpose
