Job summary

This post is based within Hawthorn Recovery Unit, which provides assessment, treatment and rehabilitation for older adults with functional mental health problems. The unit plays a key role in admission prevention for those with acute and complex mental health needs.

Main duties of the job

The post holder will hold an allocated caseload acting as named Occupational Therapist and Care Coordinator as designated, formulating care plans and risk management plans with the involvement of the client and family members.

The post holder will be responsible for delivering assessment and treatment sessions and for contributing to multidisciplinary care planning, risk assessment and discharge co-ordination.

The postholder will supervise staff and students as required, and support induction and development of staff and students within the team.

The post includes taking the in-charge role for the team for designated shifts.

Job description

As a Band 6 Occupational Therapist and Care Coordinator within the Hawthorn Recovery Unit, you will provide specialist assessment, intervention and care coordination for older adults with complex mental health needs. You will manage a designated caseload, undertaking comprehensive occupational therapy assessments, including functional and cognitive assessments, and developing person-centred care plans that promote recovery, independence and wellbeing. You will deliver and evaluate individual and group interventions, support risk management and discharge planning, and work closely with service users, carers and partner agencies to ensure coordinated and effective care.

You will play an active role in the day-to-day operation of the unit, maintaining high standards of clinical documentation, participating in care reviews and contributing to service development initiatives. You will be responsible for managing your workload effectively, responding flexibly to changing clinical priorities and supporting the delivery of safe, high-quality care in line with Trust policies and professional standards.

As a senior member of the team, you will provide clinical leadership, take charge of designated shifts, supervise and support staff and students, and contribute to training and professional development within the service. You will be expected to lead on specific areas of practice, support service improvement projects, maintain your own continuing professional development, and uphold the standards of the Health and Care Professions Council and the Royal College of Occupational Therapists.
