Job summary
Foundry Healthcare is looking for a passionate and proactive Adult Social Prescriber to support patients aged 18-50 in improving their health and wellbeing through community support, social connection and personalised care.
This is an exciting opportunity to work alongside GPs, Mental Health Practitioners and a wide range of community partners to support people facing challenges such as social isolation, anxiety, low mood, long-term health conditions and complex life circumstances.
Main duties of the job
What you'll do
As an Adult Social Prescriber, you will:
- Empower patients to take greater control of their health and wellbeing.
- Connect people with local groups, activities and support services that meet their individual needs.
- Build strong relationships across the voluntary, community and social enterprise sector.
- Develop and deliver wellbeing initiatives, social groups and community-based projects.
- Work as part of a supportive multidisciplinary team to ensure patients receive the right support at the right time.
- Help reduce health inequalities and improve outcomes through a personalised approach to care.

Foundry Healthcare is a large, successful Primary Care Network serving patients across multiple sites in Lewes and the surrounding villages.
