Job summary

The post holder will work as a Band 4 Senior Rehabilitation Assistant within the multidisciplinary Falls Service team, delivering high-quality community-based rehabilitation and falls prevention services. The role involves supporting the assessment, treatment, and rehabilitation of patients while developing the competencies to independently deliver and progress falls prevention programmes within agreed guidelines. The post holder requires a car driver and be able to drive to Barking and Dagenham, Havering, Redbridge Borough.

Main duties of the job

The successful applicant will thrive in a busy and demanding role, demonstrating strong clinical reasoning and excellent communication skills to ensure patients receive timely, holistic approach. Working autonomously within the scope of practice and under the guidance of registered clinicians when required, the post holder will assess, implement, monitor, and evaluate personalised rehabilitation and falls prevention plans, including for patients with complex physical, psychological and social needs. They will use effective communication, motivation, and problem-solving skills to achieve positive patient outcomes. Key responsibilities include delivering delegated therapy and nursing interventions, progressing treatment plans, reporting changes in patient condition, and seeking clinical advice when appropriate. The role also involves delivering education to patients, carers, and staff, facilitating individual and group rehabilitation sessions, promoting safe moving and handling practices, and providing guidance on adaptive equipment to maximise patient independence. The post holder will deliver rehabilitation in patients' homes and other community settings, ensuring informed consent, maintaining confidentiality, dignity, equality, and person-centred care at all times. They will demonstrate effective organisational skills, support the development of rehabilitation staff, ensure therapy equipment is safe and maintained, and comply with Trust policies, professional standards,

Job description

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, Goodmayes) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
