Main duties of the job

The Role

As a Community Occupational Therapist within Care Direct Plus, you will be a valued member of the team, delivering person-centered assessments and interventions within the framework of social care and health legislation.

You will manage your own caseload and work autonomously across a variety of settings, using your specialist clinical skills including moving and handling expertise to support individuals to live safely, independently, and with dignity.

Key Responsibilities

  • Carry out specialist moving and handling assessments and reviews in both community and hospital settings especially where double-handed care is provided or requested. Develop and implement effective, person centered solutions that promote independence, including specialist equipment and tailored care packages. Manage a diverse caseload, including highly complex cases, in line with relevant legislation and best practice. Work as an autonomous practitioner, demonstrating strong problem solving, clinical reasoning, and independent decision making skills. Explore innovative, collaborative ways of working across general practice, acute and community health, social care, and voluntary and private sector partners.
