Job summary

We are looking for an enthusiastic and experienced Transition Nurse/AHP to join our Living Well Services on a six-month fixed-term contract.

The role will focus on developing and strengthening transition pathways for young people moving from children's to adult palliative care services. You will work collaboratively with colleagues across Rowans Hospice and with external organisations to ensure that young people and their families experience a coordinated, person-centred transition into adult services.

The role is based at our Living Well Services Centre in Purbrook and will work closely with the Living Well Services and Community Engagement Manager, who is responsible to the Clinical Lead.

Main duties of the job

Develop and strengthen transition MDT working

  • Re-establish and develop regular multidisciplinary transition meetings with relevant children's and adult hospice services.
  • Establish appropriate MDT arrangements with Jacks Place and other partner organisations.
  • Provide opportunities for case discussion, clinical support and shared learning.
  • Support existing joint initiatives and identify opportunities for further collaborative working.

Develop a Shared Care Framework

  • Develop a shared care protocol to support young people transitioning from children's to adult palliative care services.
  • Review and improve referral pathways into Rowans Hospice.
  • Map existing transition pathways used by other specialties and services.
  • Identify and incorporate relevant best practice into a local palliative care transition pathway.
  • Produce a clear and accessible transition pathway for professionals, young people and their families.

Key Responsibilities

Experience, Qualifications, Skills and Behaviours

The successful candidate will have:

  • Excellent interpersonal skills and the ability to communicate effectively with colleagues, patients, families, volunteers and those experiencing bereavement.
  • Excellent verbal and written communication skills, including the ability to maintain comprehensive and accurate clinical records.
  • An understanding of grief and bereavement and its impact on individuals and families.
  • The ability to make appropriate decisions, recognise the limitations of the role and seek further advice when required.
  • The ability to respond positively to unexpected changes and disruption to workload or schedules.
  • The ability to work effectively in high-pressure and emotionally demanding situations.
  • The ability to manage sensitive and confidential information appropriately.
  • Effective time management, organisational skills and attention to detail.
  • Strong analytical and data-entry skills, with the ability to manage multiple tasks and competing priorities.
  • Leadership skills and experience of working effectively with external agencies and multidisciplinary teams.
  • The ability to prioritise workload and respond flexibly to changing demands.
  • The ability to share knowledge and contribute to the development and improvement of practice.
  • The ability to maintain accurate, active and contemporaneous records.
  • Well-developed personal coping strategies and resilience.

Desirable Experience and Competencies

It would be advantageous to have:

  • Experience of working within a health or social care environment.
  • Experience of supporting transition from children's to adult palliative care services.
  • Experience of organising or supporting events and engagement activities.
  • Experience of working with different IT systems and a willingness to learn new technologies and software.
  • A passion for supporting people living with life-limiting illness and their families.
  • Experience of using social media to support engagement and communication.
  • Experience of using SystmOne or a similar clinical database.
