Job summary

Responsible for the assessment, planning, implementation and evaluation of care for Cardiac Rehab patients without direct supervision, to patients, carers and relatives within a number of settings including clinics (face to face / telephone), patient homes, cardiac exercise rehabilitation groups. To support Cardiac diagnosis, symptom management and to meet secondary prevention targets. Promoting lifestyle changes and goal setting towards a healthier choice. To promote the uptake and delivery of Cardiac rehab exercise groups.

To adhere to NICE, BACPR guidance and to keep up to date with secondary prevention measures.

To work in partnership with patients and carers to offer support and advice and promote informed choice.

To work autonomously, providing effective, evidence-based care in line with competency and training records and organizational policies, procedures and guidelines.

To support integrated working with acute, PCN, social and third sectors to ensure safe, effective patient care throughout the pathway of care.

Main duties of the job

Clinical Activities

Hold and manage a caseload, assess, plan, agree, implement and evaluate care for patients.

To support specialised group and individual exercise programs under the supervision of the exercise professional leading the class.

To be able to identify patients' risk factors, to prioritise and address them appropriately in routine cases but with support of more senior staff in more complex cases.

To regularly review care delivery and documentation.

To participate in personalised care planning and promote self-care and independence.

To be competent and confident with core clinical skills (as per competency and training record). To act as a formal mentor to pre-registration nursing students and preceptees.

To liaise with and refer to other agencies and professionals as appropriate. To take full responsibility for immediate care delivery decisions.

To offer support, supervision, information and education to patients, relatives and carers and other team members.
