Job summary

The Hampshire and Southampton Learning Disability Service is seeking an enthusiastic and motivated Charge Nurse to join our Single Point of Access (SPoA) team, the gateway to our specialist service. This is an exciting opportunity to play a pivotal role in improving the health and wellbeing of adults with learning disabilities by ensuring they receive the right support, at the right time, from the right service.

Our Learning Disability Service is committed to delivering high-quality, person-centred healthcare across Hampshire and Southampton, supporting adults with learning disabilities to achieve the best possible health outcomes.

Main duties of the job

About the Role

As a valued member of our innovative and forward-thinking SPoA team, you will work alongside a vibrant rotational multi-disciplinary team, undertaking comprehensive assessments of referrals and helping determine unmet health needs that may require specialist learning disability support.

This is a varied and rewarding role where no two days are the same. You will be responsible for:

  • Completing comprehensive assessments and triage of referrals.
  • Providing expert clinical input to support decision-making and care planning.
  • Communicating effectively with patients, families, carers and professionals.
  • Working collaboratively with a wide range of internal and external partners.
  • Supporting the delivery of high-quality, responsive and accessible learning disability services.

About You

We are looking for a compassionate and skilled clinician who is

  • A confident and experienced Learning Disability Nurse (other nursing fields will be considered with evidence of learning disability practice experience).
  • An excellent communicator with strong interpersonal skills.
  • Organised, adaptable and able to manage competing priorities.
  • Experienced in assessment and clinical decision-making.
  • Proficient in IT and administrative systems.
  • Passionate about improving outcomes for people with learning disabilities.
