Job summary

Bolton Community Mental Health Services are a committed integrated service that strives toprovide quality recovery based support to people with severe and enduring mental healthrelated difficulties in Bolton 7 days per week, 365 days per year. The service calls upon awide range of evidence based holistic assessments and interventions. As a CommunityPsychiatric Nurse working in the service you will be required to undertake the role of CareCoordinator in line with CPA process, you will have a defined complex caseload which couldconsist of people who are on the Assertive Outreach pathway, and will be expected to workproactively and flexibly within a multi-disciplinary team model, and you will be expected to workoutside normal office working hours. You will also be expected to act as assessor/mentor for student nurses. This job description is not exhaustive, but is intended to give an overall picture of the role. Main duties of the job

To provide skilled mental health nursing care to people in the community. To assume the role of care coordinator for a complex caseload. To work as a member of a multi-disciplinary team. To have a theoretical and working knowledge of the Safeguarding Adults and Children's framework, and to take the lead role when necessary. To undertake initial assessments of people referred into the service, linking in directly with the Team Leader and wider MDT. To devleop implement and evaluate holistic programmes of care, including the assessment of carer's needs and assessments. To maintain close links with other statutory and non-statutory professionals and agencies, and to be able to access resources from provider services, and to maintain good channels of communication. To undertake the role of care coordinator in accordance with the philosophy of the Care Programme Approach and associated Mental Health Act (1983) legislation and Care Act legislation.
