Idle Medical Centre is seeking an enthusiastic Advanced Care Practitioner or paramedic to join our team as part of the Additional Roles Reimbursement Scheme (ARRS). Main duties of the job
This role offers the opportunity to become a part of a thriving team dedicated to clinical excellence within a positive work environment. The successful applicant will join our dedicated Housebound Visiting Team who provide support for housebound patients. One member of our team also undertakes a weekly care home ward round, thus supporting some of our most frail and medically complex patients.
Salary will be commensurate with experience, and in line with the guidance set out in the NHS England Network Contract DES Contract Specification 2026/27.
Job description
All general duties and responsibilities associated with an ACP or paramedics working within primary care.
Clinical responsibilities
In accordance with the practice rota, as agreed, the post-holder will be available to undertake a variety of duties comprising mainly visiting patients at home, but may include surgery consultations, telephone consultations and queries, care home ward rounds, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Actioning further management accordingly (arranging bloods or other investigations, referrals, prescribing as indicated, or admission if necessary).
Provide clinical support to the Practice clinical teams, answering any clinical queries regarding patients seen who may require GP/ACP/Paramedic input.
Providing counselling and health education.
Referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Expectation to complete a mandatory induction.
Compiling and issuing computer-generated acute and repeat prescriptions.
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
Communication
Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
Delivering a quality service
Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including participating in shared learning across the practice. Work as part of the team to seek feedback, continually improve the service and contribute to business planning.
Managing information
Use technology, SNOMED codes and appropriate software as an aid to planning, implementing and monitoring care, communicating information and reviewing and processing data for audit purposes.
Learning and development
Take responsibility for own learning and performance through continuous professional development, including participating in supervision, acting as a positive role model and assessing own learning needs.
Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.
Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences).

Our total patient population is approximately 12,500. With the use of effective IT systems, collaborative working, and a strong focus on education and learning, we deliver safe and high-quality care for our patients. We are a training practice and offer a number of specialist services in-house, including dermatology, diabetes, womens health as well as others, and further opportunities are available for clinical support and development. We are a friendly and supportive team, and tailored mentoring and clinical supervision will be provided for the successful applicant.
Our Team comprises 4 Partners, a Practice Manager, experienced GP and nursing teams, and our administration and reception teams. Regular team meetings take place, and a monthly team education and support meeting, as well as provision for annual mandatory training, are available.
