Job summary
The Forensic Community Transitions Pathway is a newly developed pilot service to support people in the Shropshire, Telford and Wrekin area that have left secure forensic services and are transitioning from MPFTs Forensic Intensive Recovery Support Team (FIRST) into a community mental health service. The team will provide specialist assessment, formulation and treatment to support them during this transition to gain/regain their skills and confidence to live successfully in the community.
The service is currently looking for a Senior Occupational Therapist (forensic community mental health specialism) to join the service on a temporary contract until 31/03/2028,full-time post (37.5 hours per week).
This role and service will need to develop strong working relationships with other services both internally and externally and is co-located with other mental health teams as the goals of the service are to support individuals with rehabilitation and re-integration into the community.
This is an excellent opportunity to develop leadership and service development skills and also to understand to wider health and social care system in Shropshire, Telford and Wrekin.
This post is being advertised as a temporary contract until 31/03/2028. However, secondment opportunities would be considered for internal MPFT employees upon prior agreement of your current line manager.
Main duties of the job
1. Take responsibility for the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans and when appropriate care coordination.
2. Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
3. Assess patient's individual care needs, develop, implement and evaluate programmes of care.
4.Deliver a range of non-pharmacological interventions following specific Occupational Therapy assessments to enable understanding of any expression of an unmet need and to maximise an individual's abilities.
5. Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
6. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
7. Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
