Job summary

An exciting opportunity has arisen for a Band 6 Heart Failure Clinical Nurse Specialist to join the community Heart Failure team in Pembrokeshire.

We are seeking an experienced and motivated registered nurse with strong clinical skills and a sound theoretical knowledge base in heart failure. This is a part-time post (15 hours per week, across two days).

The community Heart Failure CNS team provides specialist nursing care to individuals diagnosed with heart failure, supporting them to manage their condition effectively within their own homes. The service is focused on reducing hospital admissions, improving patient outcomes, and enhancing overall patient experience.

You will work closely with cardiology consultants, specialist nurses, primary care teams and wider community services to deliver coordinated, patient-centred care.

Strong communication, organisational and clinical skills are essential. Experience in heart failure management and/or completion of a relevant post-registration course is desirable, with opportunities for further development supported within the role.

Main duties of the job

  • Manage a defined caseload of patients with complex heart failure, using evidence-based and person-centred approaches
  • Undertake comprehensive, holistic assessments and develop, implement and evaluate care plans across the primary and secondary care interface
  • Work autonomously across a range of settings including patients' homes, clinics and hospital environments
  • Provide specialist advice, education and support to patients, families and carers to promote self-management and improve outcomes
  • Deliver education and support to colleagues to enhance knowledge and standards of heart failure care
  • Work collaboratively with multidisciplinary teams including cardiology, general practice and community services
  • Maintain accurate and timely clinical documentation in line with professional and organisational standards
  • Contribute to reducing avoidable hospital admissions through proactive management and early intervention
  • Actively engage in ongoing professional development, including undertaking relevant post-registration education
  • Demonstrate and actively promote the Health Board's values in all aspects of practice

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
