We are excited to introduce a brand-new role within our Neighbourhood Mental Health Team: New Patient Assessment Care Navigator.

This is an exciting opportunity to join a forward-thinking service focused on delivering responsive, person-centred mental health care within local communities. As one of the first points of contact for people entering our service, you will play a vital role in ensuring patients, families and carers receive a warm, supportive and efficient experience from the very beginning of their journey.

Working alongside clinicians and multidisciplinary colleagues, you will help guide individuals through the assessment and referral process, ensuring they are connected quickly and safely to the most appropriate support and treatment pathways.

Main duties of the job

The New Patient Assessment Care Navigator acts as a key link between patients, clinicians and partner agencies, supporting the coordination of new referrals into the Neighbourhood Mental Health Team.

You will support clinicians during initial assessments, gather relevant information, coordinate referrals and appointments, and provide practical and emotional support to patients and their families throughout the intake process. Your contribution will help improve patient experience, reduce delays and enhance the effectiveness of our services.

Support clinicians to complete comprehensive new patient assessments through telephone and face-to-face appointments. Gather psychosocial, demographic and physical health information to support clinical decision-making. Assist in identifying presenting concerns, mental and physical health history, risk factors and immediate support needs. Ensure assessments are completed in line with operational protocols and service standards. It is essential to hold a driver's license and access to a vehicle for community-based roles where clients visits are a part of the role.
