We are seeking an enthusiastic, experienced and motivated Lead Practice Nurse to join our friendly, well-established and supportive team.

This is an exciting opportunity for a confident Nurse Lead with previous General Practice nursing experience to lead our experienced nursing team. You will play a key role in developing and delivering high-quality nursing services, including the management of long-term conditions such as diabetes, asthma, COPD, coronary heart disease and hypertension.

The successful candidate will be an inspiring and supportive leader with excellent clinical, organisational and communication skills. You will have a good understanding of QOF, Enhanced Services, CQC standards and NICE guidance, together with the ability to mentor, develop and motivate others while helping to shape the future direction of nursing services within the practice.

30 hours a week, ideally to be worked over 4 days. Negotiable.

Main duties of the job

Main duties of the role

Working in accordance with the NMC Code, you will be responsible for ensuring the safe, effective and high-quality delivery of nursing care across the practice population, while leading the continued development of our nursing services.

You will provide leadership, support and clinical direction to the nursing team, promoting excellence in assessment, treatment, screening, health promotion and patient education. Working closely with the Practice Management Team and GP Partners, you will ensure nursing resources are effectively managed and aligned with the strategic priorities of the practice.

You will also -

Lead on the development and implementation of nursing policies, protocols and clinical standards.

Act as the Practice Infection Prevention and Control Lead, ensuring compliance with current legislation and best practice.

Support the ongoing development of the nursing team through mentoring, supervision and training.

Deliver evidence-based care for patients with long-term conditions alongside preventative health interventions and health promotion initiatives.

You will be joining a highly experienced, settled and supportive multidisciplinary team with an excellent reputation for delivering high-quality patient care.

Job summary

Working in accordance with the NMC Code of Conduct, the post-holder will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population and the development of the nursing services provided at the Practice. They will provide leadership and direction to the nursing team and develop the clinical practice of the practice nurse team in the provision of assessment, treatment, screening, health education services, and advice to patients. They will lead and manage the nursing resources, working closely with the management team and Directors to

deliver the overall practice priorities whilst providing a high standard of nursing care to patients.

Clinically, the focus is on delivery of evidence-based practice for patients with long-term chronic conditions, and management and preventative nursing interventions to all patients.

Clinical responsibilities

Structured Chronic Disease reviews including Diabetes, Asthma and COPD.

Childhood Immunisations

Cervical Smears

Contraception reviews

Other Injections and immunisations including Flu/Pneumococcal/shingles/COVID

Wound care

Leg ulcer dressings

Doppler studies/lower limb assessments

ECGs

Blood tests

Management

As a clinical member of the Management Team be responsible for the safe and effective delivery and future development of the nursing and overall clinical services.

Work with other senior staff to ensure the achievement of Practice aims and objectives and the efficient and effective running of the nursing service and the service as a whole, collaborating on setting policy for the clinical service and the nursing team.

Plan, develop and implement specific projects and new nursing services as required.

Ensure that the Nursing Team are supported with CPD meetings to disseminate information relevant to practices and ensure that the team are aware of any changes. Including gathering information, setting agendas, minuting and sending out minutes.

Lead on infection control for the practice.

Identify the nursing requirement to meet the practice population need, assessing the impact and implementation of skill-mix in the delivery of care and developing new services and increasing skill-mix as appropriate.

Maintain record of nurse competencies and ensure all teams are aware of this.

Ensure nursing team are meeting all clinical contract standards such as QOF

Liaise with nurse chronic disease leads and plan/ process map changes to improve the surgery chronic disease management ensuring we meet NICE/ QOF/contract guidelines and targets

Respond to all nurse related clinical queries from all other teams. Advise as needed.

Advise Nurse admin re clinical queries and oversee nurse rota checking correct skill mix, advising re holiday cover, absences or training requirements.

Provide clinical cover where required to maintain a safe and effective nursing service. Duties will include those typically performed by Practice Nurses/treatment room nurse as well as Chronic disease management and reviews. A particular interest in Respiratory care and Spirometry would be beneficial.

Line Management and Leadership

Clinical and operational management and leadership of the Practice Nursing Team (practice nurses and healthcare assistants), maximising the performance of individuals and the efficiency and effectiveness of the team and ensuring the delivery of a high quality service for patients.

Carry out yearly reviews of staff and conduct performance appraisal.

Carry out Revalidation interviews for Practice Nurse Team.

Maintain an open door policy for all nursing team and provide supervision on an ad hoc basis

Carry out probation interviews/ return to work interviews for all practice nurses

Build and lead a cohesive and effective team with good cross team communication.

Lead or participate in recruitment as required and implement induction programmes for new Practice nursing staff.

Organise work and delegate clearly and appropriately, assessing the competence of those taking on delegated tasks and ensuring safe practice.

Assess competence of clinical staff, including in-house certification of staff where appropriate (e.g. Healthcare Assistants).

Manage and monitor annual leave, sickness and other absence (including return to work interviews and informal counselling)

Provide pastoral support for staff when required, particularly in relation to issues that affect their ability to work.

Instigate performance management and disciplinary procedures if required alongside HR Manager.

Communicate resource constraints to the team and discuss with them ideas for effective and efficient working within these constraints.

Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.

Teaching and Training

Promote a learning environment, act as a mentor and assess all training needs for the Practice Nursing Team and organise training needed to achieve an effective and safe service.

Ensure that all required training is completed by members of the team and documented.

Plan in-house training and competencies for HCAs.

Take responsibility for own development, learning and performance including participating in clinical supervision and training as necessary to update skills and knowledge.

Carry out appraisals of the practice nursing team and update training needs as required.

Participation in the monthly educational and training events of the Practice, and set the monthly agenda for the Practice nursing team.

Organisation and delivery of the appropriate parts of educational programmes for, GP Registrars and other learners, taking the lead and/or directly supervising as required.

Participation in the induction of new staff, students and other attached professionals as necessary.

Facilitate and organise the introduction of student nurses into the practice.

Communication

Effective communication with clinical and non-clinical colleagues to ensure the smooth running of the practices.

Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.

To communicate difficult, often unpleasant or sensitive, messages to patients and families.

Act as an advocate when representing patients and colleagues.

Information Resources

Review, enter and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring, financial and audit processes.

To understand and follow the requirements of confidentiality (including the Data Protection Act) and the Freedom of Information Act and to refer on any queries as appropriate.

To follow Practice policy regarding the use of email and computer tasks as the main internal method of non-verbal communication and the use of the intranet and the internet as the main source of internal and external information, including the retrieval of relevant information for patients on their condition.

Ensure all templates, policies and procedures are up to date and align with NICE guidelines

Organisation/Quality

Recognise and work within own competence and relevant professional codes of conduct as set out by the Nursing and Midwifery Council.

To follow Practice policies, including Security and Information Governance.

The efficient and prompt delivery of administrative tasks.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Deliver care (self and team) to National Service Frameworks, NICE guidelines, PCN policy and evidence-based care.

Assess effectiveness of care delivery (self and team) through self and peer review, benchmarking and formal evaluation.

Utilise the audit cycle as a means of evaluating the quality of work, implementing improvements where required (self and team).

To participate in quality improvement initiatives, including Significant Event Analysis, peer review and the review of patient complaints.

To cooperate fully and openly with the investigation of patient complaints (or other investigations).

Respond to complaints from patients that need a clinical response or are related to nurse clinics

To participate in PCN initiatives and programmes as required.

To support the aims and objectives of the Practice and contribute to the ongoing development of the practice as required.
