Job summary

The role of the Stop Smoking Practitioner within One You Kent is to deliver highquality, evidencebased stop smoking support to residents across Kent, with a specific focus on reducing health inequalities and improving population health outcomes.

The postholder will provide targeted and equitable stop smoking interventions to priority populations, including people living in areas of deprivation, individuals with mental health needs, young people, and those with longterm conditions or recently discharged from hospital.

The practitioner will work flexibly across community, health, and outreach settings, contributing to the wider One You Kent agenda by supporting behaviour change, prevention, and early intervention. The role requires the ability to work independently while also contributing effectively within a multidisciplinary team.

Main duties of the job

The role delivers evidence-based stop smoking support to individuals across Kent, providing tailored interventions including one-to-one, group, drop-in, telephone, and digital sessions to motivate, support, and empower people to achieve sustained quit outcomes. It ensures clients are fully informed about available options within the service and are supported to make informed choices about their quit journey.

Working collaboratively with health, social care, and community partners, the post holder integrates stop smoking support into wider care pathways, with a focus on reducing health inequalities and improving outcomes across diverse communities. The role includes caseload management, accurate recording of client data and outcomes, and engagement activity to recruit and support clients through outreach and community-based initiatives.

The post holder delivers education on tobacco dependence, behaviour change, cessation methods, and harm reduction, while supporting individuals to set realistic and sustainable goals, including relapse prevention and harm minimisation where appropriate. They use health needs and inequalities data to inform practice and contribute to service development and improvement.

In addition, the role involves maintaining up-to-date knowledge of tobacco control policy and evidence-based practice, attending meetings, supervision, and training, and working collaboratively with colleagues to support effective service delivery and continuous improvement.
