Job summary

An exciting opportunity has arisen within our community therapy team to appoint an enthusiastic and motivated Occupational Therapist to join our team. This role offers a diverse range of responsibilities, a high level of autonomy, and ample opportunities for professional growth.

We are seeking an experienced Occupational Therapist to join our Integrated Community Team. In this role, you will visit patients in their homes or clinic settings, providing complex care to adults with long-term or complex health needs. You will collaborate with nursing, frailty, and Primary Care colleagues to prevent hospital admissions, support discharges, and promote self-management for patients with long-term conditions and palliative care needs. As an Occupational Therapist, you will deliver skilled interventions and conduct holistic assessments for patients in their homes.

We are seeking a skilled and motivated practitioner who is dynamic, compassionate, and committed to working as part of a multidisciplinary team to provide the best outcomes for patients. The successful candidate will possess excellent communication skills, the ability to work collaboratively, and demonstrate strong time management and prioritization abilities. The role also involves contributing to the development, training, and support of the multi-professional team and students.

Main duties of the job

To assess and treat own caseload, with supervision from a senior and to maintain own records.

To work as part of the therapy team and take part in staff meetings, and in-service training sessions.

To supervise and support students and support staff with support from Senior Therapists.

To formulate an appropriate, individual management and/or treatment programme using clinical reasoning skills and based on sound knowledge of evidence based practice; setting objectives; timed goals and ensuring treatment progression using a variety of treatment techniques.

With support and guidance from senior staff evaluate patient progress, reassess and alter treatment programmes.

To communicate effectively with other team members of the multi-disciplinary team about patients issues, in order to facilitate a team approach to patient care, specifically to actively participate in ward rounds, multi-disciplinary team meetings and case conferences.

To communicate effectively with patients and carers to maximise rehabilitation potential and ensure understanding of condition. Communication skills of motivation, persuasion and explanation, and gaining consent will be used with a wide variety of patients. Barriers to effective communication will be regularly evident, e.g. confusion, receptive and expressive dysphasia, depression, hearing and visual impairment, altered state of consciousness and perceptual and cognitive impairments. Many patients will lack the capacity to consent to treatment
