Job summary
We are looking for a motivated and compassionate Band 6 Specialist Wellbeing Nurse to join our team. This is an exciting opportunity to support people with severe mental illness to improve their physical health, wellbeing and overall quality of life. The role requires excellent clinical knowledge, strong communication skills and the ability to work collaboratively with GP practices, patients, carers and wider health and social care partners. You will provide specialist advice, undertake holistic assessments, support health promotion and contribute to the development of high-quality, person-centred care.

Main duties of the job
The successful candidate will deliver high-quality, person-centred physical health assessments and health promotion support for adults with severe mental illness, working across clinic and community settings. They will manage a delegated caseload, undertake holistic assessments, identify health needs, provide advice on lifestyle, medication side effects and physical wellbeing, and support patients to access appropriate services.
The role involves working closely with GP practices, patients, carers and wider health and social care professionals to improve outcomes and reduce health inequalities. The post holder will support safe, effective care delivery, maintain accurate records, contribute to audit and service improvement, and promote clinical governance, safeguarding and risk management.
Candidates should be confident, compassionate and able to work autonomously, with strong communication, organisational and clinical decision-making skills. They will also support colleagues through advice, supervision and leadership, helping to maintain a responsive, professional and supportive service.
Job description
Duties and Responsibilities:
Demonstrate leadership through clinical expertise, delivering high standards of person-centred care using underpinning knowledge of mental health conditions and the factors that contribute towards mental health.
Provide comprehensive physical health assessments for patients on the GP SMI register including a range of standardised screening assessments required by NHSE.
Use specialist mental health knowledge, nursing skills and experience to work safely with people with severe and enduring mental health problems identifying risk and possible safeguarding concerns and escalating appropriately as per policies and procedures.
Use specialist knowledge of mental health medications including possible side effects and monitoring requirements to assess the impact on a patients health, safety and wellbeing and to inform the individualised treatment plan.
Exercise a high degree of clinical autonomy and professional judgement to use outcomes from the comprehensive assessment to develop a personalised plan of action to ensure that patients needs are met. This may involve signposting on to other services or agencies.
Plan and organise weekly clinics for SMI patients at designated Halton GP practices in collaboration with GP administrators to ensure NHSE quarterly targets are met.
Take responsibility for own nursing equipment and supplies for clinics, ensuring where necessary, equipment checked, maintained and serviced as per Trust policy
Take responsibility for the team for stock ordering and control
Identify and keep a record of patients who have not participated in the annual physical health review and to share the information with the team and GP practices.
To review non-attenders on a quarterly basis and to employ advanced communication skills and strategies to encourage patient compliance and participation.
To work closely with local mental health services and other healthcare providers and communicate effectively to ensure that patients needs are addressed appropriately.
- Provide and receive complex and sensitive information from patients, carers or other healthcare workers and agencies.
- Plan and organise workload effectively to ensure that there is a balance between patient facing and non-patient facing activities.
- Demonstrate a flexible approach using specialist skills and knowledge of your client group to determine which patients will attend clinic and which will require a home visit or alternative approach to ensure compliance.
- Actively participate in service development and quality improvement initiatives and the promotion of the service to ensure the delivery of high quality, cost-effective care.
- To deputise for the team leader and represent the service at internal and external meetings and events as required
- Support and contribute to quantitative data collection for the service to provide evidence of productivity, outcomes, and quality and to ensure that targets are achieved.
- Support and contribute towards the development and implementation of policies and clinical guidelines using specialist knowledge to propose changes to practice and procedures in own area of expertise e.g. mental health.
- Be actively involved in the development of patient focussed education, including life-style advice, symptom management and self-care relevant to the mental health diagnosis.
- Demonstrate evidence-based practice and develop new skills in response to emerging knowledge and techniques.
- Contribute to the delivery of specialist education and training for other healthcare professionals, workers, and students.
- To participate and lead on clinical education and support for students, non-registered staff, and other members of the team.
- Maintain work related records in accordance with Trust and professional standards and guidelines.
- To maintain own competency to practice through continuous personal development and maintain portfolio which reflects this in line with NMC requirements.
- Actively participate in the Trust appraisal process, taking the lead in identifying own and others mandatory, professional, supervisory, personal development, and training needs.
- Ensure confidentiality is always maintained in accordance with good practice, Trust policy and procedures and national guidelines e.g. GDPR.
