Job summary

The Team Leader for the Children's Community Nursing Team (CCNT) is responsible for providing strategic, clinical, educational and operational leadership to the service, ensuring the delivery of safe, effective, high-quality, child and family-centred care. The post holder will lead and manage the Children's Community Nursing Team, making effective use of resources to meet service demands whilst maintaining the highest standards of patient care and professional practice.

Main duties of the job

As a key member of the Children's Services senior leadership team, the post holder will support the implementation of national policy, local priorities and Trust objectives relating to children's community health services. They will ensure that all staff work within the professional standards, codes of conduct and scope of practice defined by the Nursing and Midwifery Council (NMC).

Working collaboratively with colleagues across primary, secondary and tertiary care, the post holder will lead the delivery of community-based services for children and young people with acute, complex and chronic health care needs. They will be responsible for overseeing the coordination of complex discharge planning processes, ensuring seamless transitions from hospital to home and promoting integrated, multidisciplinary care pathways.

The Team Leader will provide expert clinical guidance, professional leadership and educational support to the nursing workforce, fostering a positive culture of learning, development and continuous improvement. They will champion evidence-based practice, quality improvement and innovation, identifying opportunities to develop the service in line with current research, best practice and the evolving needs of children, young people and their families.

Working autonomously within established Clinical Governance frameworks, the post holder will demonstrate advanced clinical expertise, sound decision-making and exceptional communication skills.

Job description

Key responsibilities include:

  • Leading and managing the day-to-day operation of the CCNT, ensuring effective use of resources and delivery of high standards of care.
  • Providing expert clinical leadership, advice and support to the team, ensuring practice complies with NMC standards, Trust policies and clinical governance requirements.
  • Coordinating complex discharge planning and working collaboratively with multi-agency partners across primary, secondary and tertiary care.
  • Developing, supporting and managing staff through recruitment, appraisal, supervision, education and training.
  • Promoting continuous quality improvement through audit, risk management, incident investigation and implementation of learning.
  • Ensuring safeguarding, infection prevention, equality, confidentiality and patient safety principles are embedded throughout the service.
  • Leading workforce development by delivering training, assessing competence and fostering a culture of learning and reflective practice.
  • Contributing to strategic service development, performance reporting and service improvement initiatives.
  • Participating in on-call arrangements and providing operational support and professional leadership across Children's Services as required.

The post holder will demonstrate advanced clinical expertise, excellent communication and leadership skills, and work autonomously to ensure the service continues to develop in line with national guidance, evidence-based practice and the needs of children, young people and their families.
