Job summary

To work within a multi-disciplinary team closely with staff from other agencies to promote and maintain high standards of care for children up to the age of 0-5 who have 2 or more areas of significant developmental delay, learning disabilities and complex needs. This will involve assessing developmental / associated health needs, planning, implementing and evaluating appropriate programmes of care and were applicable provide more specialist assessment and intervention. Such work will be underpinned by ordinary life principles and evidence based practice.

The post holder will be expected to work with children, their families, carers, staff teams and other professionals in a variety of community settings. In addition the post holder will provide management and supervision for junior colleagues and actively engage in service development activities within the team.

Main duties of the job

  • Manage a caseload of children and young people with learning disabilities and complex needs from referral to discharge.
  • Undertake specialist assessments and contribute to diagnosis, care planning and eligibility decisions.
  • Develop, deliver and evaluate evidence-based, person-centred interventions.
  • Provide advice, consultation and support to children, families, carers and multi-agency professionals.
  • Coordinate health aspects of multi-agency care plans and contribute to Continuing Care/DST processes where required.
  • Respond to crisis situations and provide timely interventions to support children and families.
  • Deliver training and group interventions to parents, carers and partner agencies.
  • Support transitions between services and promote positive outcomes for children and young people.
  • Maintain accurate clinical records and contribute to service development, quality improvement and resource development.
  • Work within a strengths-based, person-centred framework, ensuring children and families are actively involved in decision-making.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The post holder is responsible for providing specialist community learning disability nursing support to children and young people with learning disabilities, developmental delays, and complex needs.
