Job Description – Practice Nurse

Job Title: Practice Nurse
Responsible to: Practice Manager / Lead Nurse

Job Purpose:
To provide high-quality, evidence-based nursing care to patients within the practice, working collaboratively with GPs and the wider multidisciplinary team to promote health, prevent illness and manage long-term conditions.

Key Responsibilities:

• Provide general nursing care, assessment and treatment to patients in accordance with professional standards and practice policies.
• Undertake chronic disease management and reviews, including appropriate monitoring and health promotion.
• Carry out immunisations, vaccinations, cervical screening, wound care, dressings and other nursing procedures within scope of competence.
• Support the delivery of national and local healthcare programmes, including QOF and vaccination campaigns.
• Provide appropriate patient education, lifestyle advice and self-management support.
• Maintain accurate, contemporaneous clinical records using EMIS Web.
• Work collaboratively with GPs, HCAs, pharmacists and the wider practice team.
• Participate in clinical audits, significant event reviews and continuing professional development.
• Maintain professional registration and comply with NMC standards, practice policies and safeguarding requirements.

The post holder will be expected to work flexibly across practice sites where required.
