Job summary
IMP PCN are looking for a healthcare support worker to work within our ageing well team, namely the Enhanced Health in Care Homes team.
The candidate will need to be availble Monday - Friday, 22.5 hours per week over 3 days.
Main duties of the job
Main duties will include
Phlebotomy
Blood Pressures
Recording heights and weight for patients
ECGs
Foot checks
Care planning
Supporting the wider Ageing well team
Job description
The post holder will:
- Undertake routine observations including:
- Blood pressure
- Pulse
- Temperature
- Oxygen saturation
- Weight and BMI
- Blood glucose monitoring where appropriate
- Support frailty assessments and collection of clinical information.
- Assist clinicians during care home ward rounds.
- Monitor residents identified as high risk or requiring follow-up.
- Escalate any deterioration or safeguarding concerns promptly to the appropriate clinician.
- Support falls monitoring and follow-up activity.
- Assist with implementation of personalised care and support plans.
- Participate in weekly care home ward rounds.
- Support preparation of resident reviews and MDT discussions.
- Collect information required for proactive reviews.
- Maintain communication with care home staff.
- Support residents and families to understand care plans and healthcare interventions.
- Assist in identifying residents who may benefit from anticipatory care planning or ReSPECT discussions.
- Contribute to reducing avoidable hospital admissions through proactive monitoring and timely escalation.
- Provide compassionate support to residents and their families.
- Promote independence and wellbeing.
- Support hydration, nutrition and monitoring programmes where appropriate.
- Signpost patients and relatives to relevant services.
- Support personalised care approaches.
