<li>Assess, diagnose and manage patients with acute and chronic conditions</li>

<li>Prescribe independently within your scope of practice</li>

<li>Deliver high-quality, patient-centred care</li>

<li>Work closely with GPs, nurses and the wider multidisciplinary team</li>

<li>Help develop and improve services across both practices</li>

</ul>

<p>✔ Qualified Advanced Nurse Practitioner / Advanced Clinical Practitioner</p>

<p>✔ Independent Prescriber</p>

<p>✔ Experience within primary care</p>

<p>✔ Excellent clinical assessment and communication skills</p>

<p>✔ Passionate about delivering outstanding patient care</p>

<p>✔ Friendly, approachable and a strong team player</p>
