Advanced Clinical Practitioner (ACP)/ Advanced Nurse Practitioner (ANP)

Guildhall Surgery – Folkestone, Kent

Full-Time | Permanent | Salary Negotiable Depending on Experience

An Exciting Opportunity to Join Our Clinical Team which currently includes two other ACP’s.

Guildhall Surgery is seeking an experienced, enthusiastic and motivated ACP or ANP to join our established and supportive clinical team in a full-time position.

This is an excellent opportunity for an experienced ACP or ANP looking for a varied and rewarding role within a forward-thinking GP practice, combining clinical triage and the management of patients with long-term conditions.

We are a progressive and patient-focused practice, committed to developing innovative ways of delivering high-quality primary care while supporting the professional development and wellbeing of our team.

The Role

The successful candidate will play an important part in the delivery and ongoing development of our clinical services, working closely with our GPs and wider multidisciplinary team.

The role will combine two key areas of responsibility:

Clinical Triage and Patient Assessment

Guildhall Surgery operates an established total triage model, using eConsult to manage requests for GP appointments and ensure patients are directed to the most appropriate care.

The successful candidate will:

– Undertake clinical triage of patient requests, assessing clinical need and urgency.
– Make autonomous clinical decisions within their professional scope of practice.
– Direct patients to the most appropriate clinician, service or care pathway.
– Provide clinical consultations, both face-to-face and remotely, as appropriate.
– Work collaboratively with the GP team to ensure safe, effective and timely patient care.
– Contribute to the continued development and effectiveness of our triage system.

Applicants should be aware that our triage opens at 0800 hours, Monday to Friday.

Long-Term Condition Management

The post holder will also have a significant role in supporting patients living with long-term health conditions, providing continuity of care and proactive clinical management.

Responsibilities will include:

– Undertaking comprehensive assessments and reviews of patients with long-term conditions.
– Managing and monitoring patients with complex and multiple health needs.
– Developing and implementing personalised care and treatment plans.
– Supporting patients in the effective management of their conditions, promoting prevention and self-care.
– Working alongside GPs, practice nurses, clinical pharmacists and other healthcare professionals to deliver coordinated care.
– Supporting the practice in improving long-term condition outcomes and the quality of patient care.

About You

We are looking for an experienced ACP or ANP who is confident working autonomously, has excellent clinical assessment and decision-making skills, and enjoys the variety and challenges of modern general practice.

The ideal candidate will have:

– A recognised qualification in advanced clinical practice.
– Current professional registration with the appropriate regulatory body.
– Experience working at an advanced clinical level, preferably within primary care.
– An independent prescribing qualification.
– Strong clinical assessment, diagnostic and decision-making skills.
– Experience in managing patients with long-term conditions.
– Excellent communication and interpersonal skills.
– A flexible, proactive and collaborative approach to patient care.

Experience of clinical triage and using EMIS Web and eConsult would be advantageous, although appropriate support and familiarisation will be provided.

Why Join Guildhall Surgery?

Guildhall Surgery is an established practice serving a diverse patient population in Folkestone. We have a strong commitment to developing our clinical workforce, including being a GP training practice, and continually improving the services we offer.

We can offer:

– A varied and interesting clinical role combining urgent and planned care.
– The opportunity to work autonomously as part of a supportive multidisciplinary team.
– A well-established total triage system.
– Opportunities for professional development and the further development of specialist clinical interests.
– A competitive salary, dependent upon qualifications and experience.
– NHS Pension Scheme eligibility, subject to scheme rules.
– The opportunity to contribute to the future development of primary care services in Folkestone.

An Exciting Future

Guildhall Surgery is currently progressing plans to relocate to new, purpose-designed premises within the Folca development in central Folkestone.

This significant investment in local primary care will provide modern clinical facilities, improved working conditions and opportunities to expand and diversify the services we provide.

The successful candidate will therefore be joining the practice at an exciting time, with the opportunity to play a meaningful part in shaping our future clinical services.
